Forms You Might Need
- Request Prayer -- Make a confidential prayer request. If you wish to be anonymous, omit your last name and email address. The St. John's e-mail prayer team will receive your request but not your name.
- Engage with St. John’s -- Tell us a little about yourself and what interests you. We will make the necessary connections.
- Request to Receive Weekly Midweek Email Update -- an email with all the news and activities at St. John's
- Request Baptism -- Complete this form after contacting the church office (office@sjlc.com, 703-971-2210).
- Purchase Altar Flowers -- Requests for altar flowers for a weekend worship service must be received no later than two weeks prior to the requested date.
- Volunteer to Teach English -- This alerts the ESL Director who will contact you shortly. Or, email ESL@sjlc.com.
- Endowment and Memorial Grant Recommendation -- Make a recommendation on how to spend/use the proceeds of St. John's Endowment Fund. Church leaders keep a list of projects and ideas.
- Campus Maintenance Request Form -- Let the front office and maintenance team know if there is a need for a electrical, plumbing, mechanical, or general maintenance in and/or around the church campus.
- Voucher Form -- Use if you have reimbursable expenses, authorized by a Ministry Lead.
- Add Item to the Midweek Update -- Use if you want an announcement to appear in an upcoming Midweek Update. The suspense to submit each week is Monday afternoon (4pm).
